Try going back to the basics of Communication 101. Define terms. (Better yet, Draw terms, meaning on the whiteboard.) Did anyone, at any point in the definition process, ask the question "Why?" (And how many times did you ask Why?) I would submit that it is the speed of communications that is the problem here (not MVP or Hypothesis or Expectation). "Slow is Smooth. Smooth is Fast." Also, ask yourself, "To what extent did I put myself in the customer's shoes?" And, "Can I draw (not just see) the world from the customer's point of view?" Challenge, in detail, your own assumptions. This is not an Agile or Scrum or methodology issue. This is a basic Human Communications issue.
